Population Health Management
Population Health Management (PHM) is new to podiatric medicine and those affiliated healthcare providers that care for the lower extremities of patients with diabetes.
The Population Health Alliance, of which Arche Healthcare is a member, defines Population Health Management as a program which strives to address health needs at all points along the continuum of health and well-being through participation of, engagement with and targeted interventions for a population.
The goal of PHM is to maintain or improve the physical and psychosocial well-being of individuals through cost effective and tailored health solutions.
