Be More.
Saving not just limbs but lives through population health management.
Are you looking at every part of diabetes?
Most health plans and systems have programs that address multiple complications of diabetes. These include retinopathy, heart and kidney disease. However, most don’t have a program or metric that focuses on the lower extremities of people with diabetes.
Did you know that:
- Of the 31 million people with diabetes in the United States today, 25% will experience a foot ulcer in their lifetime?
- 3.8% of people who have foot ulcers will experience complications leading to an amputation?
- 65% will have a diabetic foot ulcer recurrence with 5 years?
- That there is a 45% mortality rate within 5 years of a DFU?
Our goal is to change all that.
We know that the only thing that a member sees as important is that which their physician puts in front of them.
The Arche Healthcare CLUE program is designed to engage the person with diabetes in the care of their lower extremities in a whole new way by changing their experience with their podiatrist and by equipping them with visually engaging tools to empower them to care for their feet at home.
We invite you to Be More and join us in making this change.
Goethe said, “Knowing is not enough; we must apply. Willing is not good enough; we must do.” We are equipped and ready to do. Are you?
